Explore reports

Open Form Responses, choose a template and reporting dates, and inspect the matching records before exporting.

The Reports hub lists the reports your account can run. Start with the question you want answered — this one is which visit notes were submitted this month.

Opening a report gives you a setup page, not results. You need access to the template and its responses, and data in the period you choose.

An empty result reflects your filters and access. It is not proof nobody did the work.

  1. Step 1

    Open Reports

    In the side menu, click Reports. It opens to list the reports you can run — which ones you see depends on your account and enabled features.

  2. Step 2

    Choose the report and template

    Open Form Responses and select the visit-note template. The Form Response Export setup page starts with no template selected — it is not yet a report.

  3. Step 3

    Set the dates

    Under Filter Date By, select Submitted Date, then set Start Date and End Date to this month. Form Date and Last Updated Date answer different questions, so check which one you picked.

  4. Step 4

    Read the results

    Wait for the responses to load, expand the entity group if you need to, and read the subjects, submitted dates and statuses. Drafts are not included here.

  5. Step 5

    If nothing matches

    Work through it in order: is a template selected, is Filter Date By set to the field you meant, is the range wide enough, and does your access cover the clients the responses are about? Drafts are stripped from this report entirely, so an unfinished response will never appear here however wide you set the dates — look for those in the Forms Browser instead.   6. Step 6

    Which date the range is measured against

    Filter Date By starts empty and stays disabled until you pick a template, because the choices depend on it. Submitted Date is when the response was sent in. Last Updated Date is when it was last touched, which moves every time somebody edits it. Form Date is a date question inside the form itself — the visit date, say — and it only appears when the template has one set as its date field. Asking "what happened in March" and "what was submitted in March" are different questions, and this control is where you choose between them.

    Other ways in

    Form Date and Last Updated Date answer different questions from Submitted Date, and the Forms Browser gives a quicker read for a single template without generating a file.
